In-Office Wellness Plan

Introducing our in-office dental plan: an annual saving program designed for families and individuals that provides access to top-notch dental services at a significantly lower cost. This plan is ideal for those without dental insurance, anyone unhappy with their current insurance plan and would like an alternative option to traditional benefit plans, or anyone looking to take advantage of significant savings. Unlike traditional insurance plans, our savings plan features no deductibles, no frequency limitations, no maximums, and no waiting periods to start treatment.

How Can I Qualify?

Any patient of Friendly Dental Care may purchase the savings plan who does not have an active insurance plan. Patients whose insurance does not accept assignment of benefit (pays the patient directly instead of the office) will be eligible to purchase the plan. Patients will be eligible to start using the benefits immediately!

Covered Benefits Include

Adult Preventive Plan ($450) and
Child Preventive Plan ($350)

1 comprehensive exam

1 periodic exam

2 prophylaxis (cleanings)

Diagnostic X-rays: 1 set of Bitewings per year, 1 Full Mouth Series and Pano every 3 years and PA X-rays as necessary.

2 Fluoride treatments

2 Periodontal exams

2 Cancer screenings

Periodontal Plan ($750)

If you are diagnosed with periodontal disease you will need to upgrade to this plan to receive the proper level of care, it includes all the same yearly diagnostic X-rays, exams, fluoride treatments and cancer screenings as the preventive plans as well as:

Full mouth debridement if necessary

4 yearly maintenance cleanings after Scaling and Root Planing has been completed (separate charge per quadrant of SRP). (3 maintenance appts the first year after SRP, then 4 maintenance appts in subsequent years)

Excluded From the Plan

  • Services which, in the professional judgment of the attending dentist are neither necessary nor recommended for the patient’s dental health.
  • Loss, theft or neglect on the part of the patient requiring replacement of crowns, bridges, dentures, removable partials, bite guards or other dental appliances (sleep apnea or essix retainers).
  • Elective procedures such as cosmetic bonding or veneers.
  • Analgesic services including Oral Conscious Sedation and Nitrous Oxide.
  • Products sold within the office including but not limited to: Fluoridex, Chlorhexidine, toothbrushes, whitening kits and refills, OraCare.
  • Unused cleanings or periodontal maintenance appts are not allowed to be carried over into the following year. Members must take advantage of the plan benefits within each applicable 12 month period or you lose them.
  • Any other services completed after the plan coverage has lapsed.

Payment

  • Full membership fee is collected upfront to enroll. (perio plan upgraded after diagnosis)
  • There is no setup fee for our plan.
  • Plan benefit savings begin immediately upon registration and remain in force for a period of one year.
  • Coverage will automatically renew on the anniversary of your initial coverage date unless either the participant or Friendly Dental Care gives 30 days notice to the other party.
  • Patients may use CareCredit to pay for services performed in the office, but may not use them to pay for membership fees.
